Our Story
Better health often begins before illness.
Living Informed for Tomorrow Foundation (LIFT Foundation) grew from a simple belief: people should have the knowledge and opportunity to take action for their health earlier.
Why LIFT?
Too many people learn about health risks after they have progressed. Others have limited access to practical health information, opportunities for screening or clear guidance on what to do next.
Starting in Wakiso District, we work alongside communities, Village Health Teams, community health champions and health facilities to bring prevention and early action closer.
Our Vision
Healthier communities where people understand their health, recognize risks and have opportunities to act early.
Our Mission
To promote healthier communities through practical health education, prevention, early detection, referral partnerships and community-led action.
Our Values
Compassion, Prevention, Empowerment, Partnership, Integrity, and Innovation.
Our Approach
Understand → Prevent → Check → Connect → Follow up
What Guides Us
Our approach is a simple pathway connecting health knowledge with prevention, screening and appropriate care.
Village Health Teams
We work alongside VHTs to connect health information and prevention with households and communities.
Community Health Champions
We are developing a network of trusted community members who can share health information, encourage prevention and support early action.
Health Facilities
Our partnership with a public health facility helps connect community-level health promotion with appropriate care.
Communities
We listen to communities, learn about their health concerns, and use their experience to shape our work.
Our Current Work
LIFT is currently undertaking hypertension and diabetes awareness and screening in Wakiso District. This work is helping us understand the barriers people face in accessing preventive healthcare and the value of bringing health information and early action closer to where people live and work.
Our early experience is also helping us build stronger relationships with Village Health Teams, community members and our public health facility partner.
